| | A | The Client Demographic form has been replaced by the PCP Consumer Admission form. You can access it in the SMC BUI system by first logging into the SMC Extranet: http://extranet.smokymountaincenter.com/. Once you are logged into the SMC Extranet, click on the SMC BUI link and log into the BUI system. When you get logged in, click on menu item 11 (Admissions and Discharges). Click on the Specify Client item in the left menu pane. Once you have your client specified, click the Admission Screen item in the left menu pane and begin filling out the form. |
